  • Bowel/bladder routines (catheter, stoma, continence plan)
  • Feeding plan & any swallowing risks; allergies
  • Seizure or behaviour support plan; triggers & calming strategies
  • Daily schedule goals (school/work/therapy/community); transport needs
  • Preferred languages & communication supports (AAC/PECS/devices)
Home setup & equipment

We can advise on ramps, grab bars, shower chairs, transfer boards, pressure-relief cushions/mattresses, and safe-path layouts.

Clinical scope: Care workers handle non-clinical tasks and reminders. Nurses perform clinical care per training and doctor’s orders.
